2010/2011 Enrollment Forms
Enrollment Forms for the 2010/2011 school year -
Families already registered for the 2010-2011 school year must complete the following forms and turn them in before your child begins the school year. The forms can be turned in at the Parent Meeting on August 26th at 7pm. The Parent Meeting will be held at Our Lady of Perpetual Help Church in Aurora.
***Please note***
1. The Child Enrollment and Health Information sheets asks for at least one emergency contact person, for the NAEYC we need two emergency contacts listed.
2. Two of the forms are optional - The Medical/Physical Care Plan form & The Request for Medicine form.
3. Please fill out forms completely
